The Credit Verification Service has been completed.  To enroll in the program, you must first download a copy of the CVS Policy and read it in its entirety.  The Validator that you would like to use will also need to read the policy and send the CVS Validator Agreement to:  MDHSA, 32 West Main Street #5, Waynesboro, PA  17268.

NOTE:  This program is NOT for MDHSA Diploma Program students.  This program is ONLY for students receiving a parent issued diploma (usually for students from states that do not have a diploma program.)

All the forms that you need for this service are listed below as well as on the Forms and Applications page.  Please note that you can only use the Credit Verification Forms for this service - MDHSA Diploma Program forms will NOT be accepted for this program.

Credit Verification Service Forms are as follows:

          To enroll in the program and to file your annual report:  CVS Enrollment and Annual Filing Form

          To report the courses that are approved by the Validator (a separate form for each year):  CVS Credit Verification Report

          To report courses to be transferred from a school or other institution (official transcipt is required):  CVS Transfer Courses Form

          To request your final transcript:  CVS Final Graduation Report for Official Transcript

          To request an additonal Transcript:  CVS Request for Transcript

          To enroll as a Validator:  CVS Validator Agreement
